Penn Manor Health Services requires a form to be filled out in order to complete your student registration.

Your child's immunization dates are essential for registration. To help you determine if your child has had the minimum required immunizations, we have included a worksheet. Please complete this and bring it and an up-to-date record of immunizations to registration. If you do not have a record at home, please call your doctor or health center to get a copy of the current dates so you can bring it with you to register your child.

We hope that completing these forms before registration will help to expedite the process. If you have any questions regarding these forms, please feel free to call the school building closest to you and consult the school nurse.
