Immunization Information Sources


Immunization Requirements
Diphtheria & Tetanus (DTap, DTP, Td, or DT)

Grades K-8: Four properly spaced doses with one of the doses administered on or after the fourth birthday

Grades 9-12: Three properly spaced doses.

Polio

Three doses (IPV or OPV)

Measles

Two properly spaced doses with the first dose given at 12 months of age or older

(The Department of Health recommends the combined MMRII vaccine).

Mumps

One dose administered at twelve months of age or older.

(Department of Health recommends the combined MMRII).

Rubella

One dose administered at twelve months of age or older.

Hepatitis B

Grades K-10: Three properly spaced doses

Varicella

Grades K-2 & 7-9 One dose

Lancaster General Child Protect Program

Lancaster General Hospital offers an Immunization Program for children and youth up to age 20. The immunizations are provided free of charge for those children who are not covered by health insurance or whose insurance does not cover the cost of immunizations. Every Thursday from 8:30 - 11:00 A.M. and 1:30-2:30 P.M. You must call for an appointment. A $5.00 per child maximum charge. This must be a check or money order. No cash is accepted. You must bring a copy of your child's immunization record. The child must be accompanied by a parent or a legal guardian . Hepatitis B and Varicella vaccine immunizations will be given to children up to and including the age of eighteen.
